Holly’s Story

Sarah’s daughter Holly had recently moved back home after her relationship had broken down. Holly wanted to get fitter and was feeling a little isolated and Sarah was worried about her daughter’s physical and mental health.

Sarah and Holly were in their local library and saw the Connect Well Desk. Holly spoke to the Connect Well social prescribing champion and was shown some ideas of local projects she could get involved in.

Holly had always loved gardening and so asked to be connected to the local community gardening scheme.

Six months on, Holly is a regular volunteer at the scheme and has made new friends because of her time with the project. She feels happier and has lost some weight through her physical efforts working the plots and planting. Holly is actively promoting the benefits of the scheme to other friends and the new people she meets.

L and M’s Story

Mother and daughter, L and M, accessed the My Social Prescription Scheme online.

M contacted My Social Prescription in Colchester when her mother was diagnosed with a health condition that affected her ability to travel alone and live independently. M lived abroad and wanted to make sure that her mother was safe and supported by people locally.

The team found volunteers in the Time Bank, local groups, such as Age UK, and transportation to help her to live healthily and happily and her daughter was reassured.
