Young Adult Palliative Care
The J’s Hospice provides care and support for 16-40 year olds with life-limiting/life-threatening conditions. The J’s Specialist Nurses provide care to ensure a successful transition from children’s to adult services, offer effective symptom control to avoid unnecessary hospital admissions and devise plans of care in conjunction with patients to ensure needs are met. End of life care is provided, in recognising dying and helping young adults prepare through advance care planning and providing practical and emotional support. Bereavement support is available through our Social Worker, available after the death of a loved one and is offered on an individual or group basis. J’s Hospice Carers provide respite care in the home which helps families feel supported and reduces strain from the constant cycle of care that is often needed. Through social opportunities and interaction the carers can improve the wellbeing of young adults which may enhance life expectancy. Counselling and Music Therapy is offered to patients and families as an opportunity to share thoughts and explore feelings with a qualified professional and Chaplaincy provides spiritual support to everyone regardless of their religion or beliefs. Complementary and beauty therapies are provided to ease anxiety, aid symptom control and encourage relaxation. Volunteer drivers offer the use of The J’s Hospice mobility vehicle to patients attending hospital appointments, accessing their community for social activity or the J’s Inspire! social group. The Inspire! group is a vibrant, busy meeting. It provides young adults some time away from families to have fun and socialise with people their own age, whilst enabling their carers to have some respite. The focus of The Js’ Hospice care is on parallel planning—to plan for both living and dying to ensure both are achieved with maximum choice, wellbeing and support.
