Connect Well Essex to add value to ‘My Social Prescription’ in North East Essex

Do you want to feel healthier, happier and more involved in your community?

Launched in 2013, ‘My Social Prescription’ is a community based scheme led by Colchester Community Voluntary Services to connect community members to voluntary and community services and volunteers to improve health and wellbeing.

The scheme operates across North East Essex in Colchester and Tendring. It has been designed to reduce time spent finding the help people need by informing them of the right service, in the right place, first time.

From Autumn 2015, the project will be utilizing Connect Well Essex to make direct referrals to local groups. The scheme has successfully been working with EssexConnects for more than 2 years to make dozens of links between individuals and groups. The connections include befriending services, carers organisations, hobbies and support agencies.

This is achieved by identifying the most appropriate referral, utilising community assets (i.e. people, services, community resources) to respond to the social need, ultimately improving the quality of life of the patient and family and reducing demand on public services. It also reduces duplication of effort, plus encourages independence and self-care.
